I recently noticed that when there's a new post on a thread I'm following, I might not get a notification. sometimes I do, sometimes I don't. it seems random. I did not switch off notifications for those threads. it's even happening with one of my own -- I missed a few comments as a result! very frustrating.

I'm using the Chrome browser on a windows PC.
