These were made by our local glass fabricator. It is just plate glass with 2" beveled edge. Any local glass copy should be able to do this for you if you show them the photo. :)

I do not. We would have measured on site to fill out the space for symmetry. Would just depend on your space and what it will allow. I like to take blue tape and outline mirror before deciding on size after lights have been installed. Hope this helps.
