I would not be concerned unless it gets worse. All concrete cracks, just a matter of when. Added concrete does not have the advatange of being on freshly compacted soil. It is a patch and not as stable as the original was so the cracks will most likely appear sooner which is what you have here.

They installed a thick vapor barrier, but no rebar under the replaced concrete. will this be a problem for us? (this secrion of the floor has been redone 4 times now, and we arent sure if we need to insist on a 5th or if we just move forward wirh the flooring installation)

You evidently have more patience than Job to alow 4 failed repairs and considering a fifth. Once is enough for me and if more people took that stance dwaddling contractors would be driven out of business.

You did not mention previously that you were having flooring installed over the channel patch. Let them try again, maybe the 5th time will be the charm, as you cannot monitor it with flooring over it until the flooring fails. Or find someone who knows what they are doing.

This was our third company. seems like everyone we hire wants to take shortcuts. havent been able to find a reliable contractor in our area for years.

That's because the reliable knowledgeable contractor knows how much money it takes to do the job properly and everyone hires low bidder. If the marketplace is not providing reliable contractors, consumers are getting just what they're willing to pay for.

We did not go cheap. this was one of the larger foundation and waterproofimg companies on the east coast. this is why we allowed the redos- we noticed the issues after the initial work completion and therefore had already paid them.

This makes these guys look very bad since, they did not treat it like a normal foundation job, even though it is a repair. Based on the size of the opening. It needs anchors, rebars and wire mesh. I am pretty sure they would not pour footings without including those items.

If, you live in a cold climate area. you can be dealing with frost heaves causing cracks.
