I would see about removing the shoe molding that is against the front and sides of the fireplace, before I did anything with it. The shoe molding does not look good. I realize that things can be different in person, but, in the picture, the fireplace itself is beautiful. I would clean it good, and get some decor on it and above it, and see then, how it looks. Right now, it is there just naked, but, decor, will give a different look entirely. You can always do something else with it later. I wouldnt be too hasty with paint. Once you paint it, it is a huge job to strip it back, and it may never look right again. No tv above it. A nice painting or even a mirror if there is something nice to show up in the mirror. A painting is usually better.
