As already commented above, there is no reason that the built-in unit has to have the tv in the center. You should design it based upon all of your needs (for example, do you need closed storage vs. open shelves?) in addition to where the tv will be located.

With respect to adding a fireplace (+ where it should be located), you should do a search in order to see if you like any of the photos that you find.

You also could use AI to have some quick renderings made showing you different options. Once you see some options, you will be able to decide which one you like the most.
