We found the bench at a local antique shop and the shelf was made by our carpenter. Originally they were both dark wood which we later sanded & painted white. Not every store will have what you're looking for so we used a bit of our imagination to turn them into cohesive pieces.

How wide is the bench? I need a narrow bench to place in a narrow entry way and this seems like it would be a perfect width without taking up too much space in an already narrow space...

We actually had the bench custom-made by a carpenter, but it's a little over 48"long and 12" wide. It was originally dark-wood that we re-finished in a white paint.
