Tile Backsplash above butcherblock

We decided to rip out our beadboard backsplash and install subway tile. Question: Where should we start the install of the subway tile? Most tutorial show a spacer flush with the countertop then caulking it, but we just don't have that "flush-ness" to it. When we were going to keep the beadboard, we bought 1/4 round moulding to fit up against the backsplash. We can still do this, but the question remains, where do we line up that first tile? (Also, how much do I need to sand down this texture on the walls, or at all?)
