Hi mpguy, This home was built many years ago and at the time the General Contractor purchased the vents. As a result I have no way of knowing who from. Fast forward to today, you can call Concord sheet metal https://concordsheetmetal.com/pictures/dormer-vents/

or do a web search of 'residential arched copper dormer attic vent' and you'll immediately have several other vendors to choose from. Example:
