a couple inches above, modifying as needed for light fixture shape and orientation (and sometimes mirror orientation). there's not a strict rule -- often you're constrained by headers that affect where the junction box can be.

Thanks for the replies and for the photos. Do you think it is okay for the shades of the lights to hang down over the mirror or place the fixture high enough that the bottom of the shade sits about the mirror? Our shades will face downward for better lighting.
