Last pic looks like the branch just about got broke off and might be starting to repair itself, this could be pruned back. Second last pic, the tree has covered a couple of pruned branches but not sure about the lighter coloured bark lower down, but be just new bark exposed from the girth of the trunk growing. Third from last pic worries me, does not look good. Other pic are fine, leaves get chewed on and attacked by all sorts of stuff and by this time of year can look crappy. Can you get a better picture of third from last pic area. Some trees tend to set fruit biannually especially if not thinned in a heavy fruit set year or frost when blooming or...

It looks like apple anthracnose - there is a distinctive "bull's eye" marking on the leaf in the second photo that is very characteristic of this pathogen, as are the cankers. I'd do some research on the topic - it is common on parts of the west coast but has also been reported in New England, MI and Canada - or consult with your local extension service to confirm a diagnosis and for control measures appropriate to your area.

Gardengal has the right idea most city/counties have an agricultural dept. that will assist you. Your tree is not young, the diameter of the trunk tells me so.

My guess is the tree has been neglected recently. It needs pruned and I'll guess fertilized too. You will need to do a couple hours reading on the care.

thinning of fruit at an early stage is important for apples too, if not next years crop will suffer.

And I suspect that you must routinely apply a doamant spray, possibly other sprays, during the growing season.
