You have to tie the tomato plants to the stakes and cages. They don't hold on by themselves. I use simple twist ties from the supermarket. Once they get much larger than the stakes or cages, all bets are off, though.

And you have to have the stakes deep enough in the ground to support the plant. Same goes for cages. Post a picture so we can see what the problem is.
