Modern Window Trim Help

I like the door trim in the attached picture. My guess is its a 1x4 on the side and 1x6 on the top, but not sure. Also don't know what is the thin molding around it. Does anyone have any idea how to get this look? Can i buy this in one piece anywhere? Really appreciate any advice as my trim work starts next week.

Looks more like 1x4 & 1x6 boards that had an edge bead added to the board. I do not know of anyone that has that as an off the shelf casing.

Millworkman how would they add the bead to the end of the head casing? Wouldn’t it be easier to mill the bead as a separate piece and install it after the 1/4 and 1/6 are installed (almost like backband)

To me it looks like a casing with a bead. The flat area between the casing and door is the "reveal" of the door jamb. Here is an example, except here they place the bead of the casing next to the door. http://www.vintagemouldings.ca/prodDetail.php?prod=271

$$$ - hard to do right to get a perfect crisp 45 degree cut on the smaller piece - will need to be tacked and caulked to the larger 1x to have it look consistent. (And all I keep thinking about is the dust gradually getting into the crevice.

If you don't have a REALLY GOOD trim carpenter, I would not recommend this.
