38" is not really enough for a pivot door at the entry. For a typical hinged door, you need a 36" entry door (this is the actual door - the rough opening will be wider). For a pivot, you'd want an equivalent clear opening width plus the width of the backswing portion. I think a 48" wide door with the pivot hinge 12" in is about as narrow as you'd want to go and still get the effect and feeling you're looking for.
