vanity needs to be flush to wall?

I'm doing a bathroom. Current built in vanity is flush with left wall. On right side is toilet. If I buy a ready made vanity, I have option to make countertop with no lip on left side so can be flush to wall. Or if I don't do that, how far should it be off of wall so it doesn't look like I'm trying to make it look built in, pushed against wall ?

personally I will go with built in look, the gap will create a nightmare for cleaning
