Can Anyone identify this valve? I think it's a Kohler bathtub. The bathtub has jets and I'm also including pictures of the plumbing in case that helps. I have been to 2 plumbing supply companies in my area and they are unable to identify this part or even a replacement part.

In the pictures, there are valves in there now but they did not fit properly and ended up leaking after about 30 days.
