IMO the bedding does nothing for the style of headboard you have so I would start with new bedding and as for rug I would do one on each side of the bed unless the room is huge for me the whole point of a rug in the bedroom is a nice spot for your bare feet to land on

You have wall to wall now? I wouldn’t layer another rug. It will not lay flat and as a result a good chance of a trip/fall.

Agree with Patricia that the duvet doesn’t do justice to the headboard (or the other way around), just too many dots.

I like this one because the pillows connect to the headboard wall and it is dressier.

At our new place, it is a big room with hardwood floors-- that's why I was thinking about a rug!

Some serenity in the bedroom is likely needed for your household. Don't add distracting pops of color, just contrasting colors.
