It's a small, tight space, used about as well as it can be. I don't know what you want to achieve -- you'll spend a fortune to add a foot, will have to shift all the plumbing, and about all that can be changed is squeezing in a wider vanity, which seems unlikely to be worth the 5-figure pricetag...

where would you put a window? what creates the angled change between vanity wall and toilet wall?

A window is always a good thing. And a recessed medicine cabinet and good lighting always help small spaces.

If you add a window - where will your mirror/medicine cabinet go? A window above a toilet presents a privacy issue - however - there's lots of dilemmas where shutters are recommended.

Is there a tub in another bathroom that you can use?

What is your main issue? Do you just feel it is unattractive? Or is there functionally something not working for you? What sort of style do you like?

How about adding a small outdoor jacuzzi?...always hot and ready...they come in a bathtub size that is easy to install...just plug in and fill...much easier than filling a whole tub every time....that then starts cooling off as soon as you get in...

If you can add a window and extend the wall out near toilet without any problem, then do it- at least the window would bring in light and the extension will add more room, keep the tub though. Consider adding handles on the wall and near toilet to help you with standing etc. if you need it- for safety reasons.
