Apple tree disease and treatment?

I have an apple tree we have built our deck around, the tree may have been planted in the 1960s (bought 1950s house from original owners children) i have been treating it for powdery mildew as well as will aphids. It seems to be slightly healthier than previous years but I am unsure how/if I can cure the tree and bring it back to full health. Can someone please confirm what disease the tree has and the best way to treat it. I REALLY want to save this tree. I have photos but not sure how to attach?
