It’s a Rhododendron. I hope someone else can suggest a cultivar name.

I did not realize some of them bloom in the Autumn, also did not know Rhododendrons undergo foliage color changes. Would this be deciduous or semii-deciduous.

I would guess Olga Mezitt or another of the Weston PJM hybrids. They will often throw a few flowers in the fall if conditions are right and they DO go through rather significant fall/winter color changes, with Olga offering maybe the closest to what's shown in the photo. I would also consider some of the yellow/dropped foliage to be a water issue. Looks to have been malpruned into a tight bun as well.

Water issue, interesting. We have had 35 inches of rain since July 1.

35 inches??!! Well, that could certainly account for it. We often don't get more than that all year :-)

Walk past again when the Skimmias either side are in bloom. They smell fabulous in mid winter.
