Need help placing the cable box for my tv over the fireplace

My tv is over my fireplace as depicted in this photo . My mantel is short depth wise, where is the box in this photo & how does the remote control send the signal to the box?

    i believe the cables are ran behind, cable box should be in the shelving at the right

    Yes, it's all behind the scenes, the shelves, fireplace mantle etc were custom designed with hideen outlets and all cables run in a channel behind the cabinetry.
