Need bead board help!

We are renovating the main bathroom in our 130-year old house. We'll be installing bead board on the bottom part of the walls and must decide how high it should go. I have read it should go 2/3 of the way up the wall. Is that what you all generally do?

I also have concerns about the trim at the top of the bead board. I will be hanging a large mirror over the sink instead of a medicine cabinet. It seems like the trim above the bead board sticking out would interfere with the way the mirror hangs. Have any of you found this to be a problem?

What kind of trim is usually used at the top of bead board? I am picturing some type of chair rail trim, but perhaps there are other choices. Have any of you used something else that would be flatter? If so, then I suppose that would help with the mirror problem.
