What panel? Removing the panels would not expose the subfloor, only removing the frame would show that. The zoomed in pics from 3" away make it next to impossible to tell what you have going on.

I see nothing wrong there. The foam will not fill every tiny bit on the outside edges.

Again. No. The spray foam underneath the door sill should have filled all the voids. Small voids at the edges are not an issue.

Thank you for your advice. What kind of professional should I be calling? The reason I decided to take a look underneath is because I suspect moisture/humidity issue. I keep finding tiny insects that are apparently attracted to moisture (and new homes) I keep telling my condo association but I guess because no water is actually seen they don’t take it seriously. Would you know the most common source that moisture or water would enter? I thought maybe on each side. The patio doors give access to a cement balcony. It’s only a 4 year old building. Thanks
