For sure it was custom, so someone spent some time and effort into designing and executing this millwork detail.

This design would be appropriate for a home that had Shaker cabinets and or doors in the home.

A carpenter could fill them in with wood and sand it down smooth ready to be repainted.

Thanks Ms Beverly! My fireplace is offset with the left side being twice as long as the right. I looked up suggestions online & saw some designers putting an entertainment center on the long side to take up some of the extra space, but I don't know what style would go with the current fireplace trim.
