Hi Leslie, As you can see from Forma's response, this was a custom design. When I first contacted them a year and a half ago, I wanted something similar but without the significant costs. Had an arched awning made and we love it. It created a 'shelter' of sorts at the front door, and gave us the modernist traditional look we were going for. Since this is not our forever home, the cost justified the means.
