Where do you get the glass in the transom? Is it custom? how would you describe it?

That looks like a new version of leaded glass windows common in craftsman bungalows. I have them in my house built in 1910. This page shows a similar window in a 1920 house: http://sohosandiego.org/reflections/2008_34/preserving.htm

A stained glass artist could surely make you one.
