The spots are likely cedar-apple rust, which won't kill the tree, although it will defoliate early and look lousy. Worse in some years than others. You can spray to prevent it. Can't say about the bark without a pic.

That said, I did recently remove a pear tree with rust. I don't like chems and have 4 little kids and a dog. It dropped leaves and misshaped little pears which attracted yellow-jackets. I planted a witchhazel instead. Afaic, there are too many wonderful disease-free trees out there to take up the room by something so high-maintenance for so little benefit.

IMHO ... be done with it ....

there are newer varieties that are nearly resistant to the rust... with no help from you ... nor exposure ....

and a heck of a lot of better plant for the space ....

Yeah, I'm not into chemicals. It's an ugly, overgrown tree in a weird spot, and fruit tree wood burns really nice. Think I'll replace it with a nice Serviceberry for the birds.
