If you are not looking for really rough wood, then there are various companies that will custom make. Look around your area for custom millwork companies. One company that makes doors like the one above is Trustile.

You can look at Trustile online, but I would not order anything without talking to them in person and making sure of what you are ordering. They are pricey and you don't want something to go awry with the order. Good luck.
