Hornworms or tomato fruit worms (aka corn borers) are the most common culprits. But the treatment is always the same as for all caterpillar pests - Bt dust or spray.

In our area, armyworm is also a common caterpillar that does that kind of damage. But as Dave says, the solution for caterpillars is BT.

That looks like slugs the hornworm, as far as I now likes the leaves.

Hornworms do eat fruit all the time. Although the large ones don't tunnel--they just eat the fruit wall.

I've never had slugs attack fruit that wasn't down near the ground. I suppose in very wet climates, it's certainly possible. But slug damage tends to have less surgical margins, in my experience.

Depends on where the egg is when the larvae hatches and that is often on a fruit. While still young and small the just hatched hornworms often eat right into the fruit. And the adults, like gb said, will eat anything they can reach including fruit.
