Is this for future-proofing or current needs? Not all wheelchairs are created equal and powered electric chairs have a different turning radius than a manual Quickie chair. I would want to know that to incorporate into the design. Will you need grab bars or just a place to sidle up to the loo for transfer? Will the shower have a built in bench or will you have a removable shower chair?

if you consider a zero threshold shower with a glass partition that swings out of the way, next to the toilet, you can use that area to get to the side of the toilet

Good point - slightly modeled after my Mom's bathroom in rehab. Perhaps move to a single vanity and put it along the same wall as the toilet. Install grab bar in a pony wall AND the TP holder. the open shower is then your side access

I have to believe that the additional maneuvering room in an accessible bathroom would be more appreciated than 2 sinks.

If I needed a home that was fully wheelchair accessible or was remodeling to make something this way, I'd hire an architect with experience in doing just this. Wheel chairs take up a LOT of room and their maneuverability is often limited except in the very, very most expensive ones, typically used by otherwise fit, young paraplegics.

Mark's sketch works as long as there is enough room to park the wheelchair between the toilet and the shower. You will also need to decide between roll in shower and transfer shower as they have different dimensions, control placement etc. Every door should be 36" with enough room around it for the person in the wheelchair to open it.

Most accessible designs must accommodate a range of physical limitations or rely on assumptions about the typical user. Assuming this space is for a particular individual with known physical limitations, is side transfer to the toilet desired now or in the future? Will transfer at the shower be desired? Is assistance desired now or in the future?

Based upon my own experience of being in a wheelchair, don't put thick pads under carpets.
