This is a contractor's cost take-off not a design specification. A description of the materials and how they would be installed would be helpful.

One of the areas that can be most problematic is conditioned living space over a garage.

From what I can infer they are planning to use spray foam insulation under the floor, on the walls and ceiling. I suggest special attention to this area with respect to the materials and installed R-value. I also suggest you consider zoning the 2nd floor HVAC system so the area over the garage can have it's own temperature control. Your builder should be able to help guide you regarding the best options to do this. If you're acting as your own general contractor, a HERS rater would be a good resource to consult before you commit to a particular insulation or HVAC strategy.
