Builder Danger Sign?

Found a builder we were happy with for a kitchen remodel. Their designer came up to firm up the plans. Her design had some problems:

* The kitchen island was flipped 180 from where it should have faced

* She put two sets of drawers in a place that clearly only fits one

* The one must-have feature we discussed wasn't included

To me, that seems like a ... monumental fail? If they're making obvious, big mistakes in the design - how many little mistakes are hiding in there that I don't even know about?

Is this a clear sign to run far, far away from this builder? Is this normal? Give them a second chance (they have good Yelp and Google reviews)?

The first builder we were going with we ended up firing - final estimate was 3x initial estimate with no warnings along the way. Starting over yet again is not appealing, but this remodel is too dang expensive not to get right.
