NEW BUILD: Spalling foundation? Can someone help identify?

Hello! We just moved into our new build in December, 2019. We've recognized issues with the patio falling apart so we have the concrete company coming back to completely remove and redo the entire patio.
NOW, I am seeing what looks like spalling on the foundation as well on the inside and outside of the garage. What the hell is going on?
