The shower curtain rail came with a separate bracket that holds the hand held shower. Bought it from Signature Hardware

Hi David, Lisa from Signature Hardware here. It's difficult to tell how the handheld shower is connected in that image, but it might be a hand shower bracket like this one: http://www.signaturehardware.com/product2800. If you have a wall close enough, you can use a wall-mount handheld shower cradle: http://www.signaturehardware.com/product23132. No wall? Try a riser pipe hand shower support: http://www.signaturehardware.com/product590.
