Most masonry contractors who do restoration will be able to help resolve your smoky fireplace. If it tends to smoke at the beginning of a fireing you may just need to prime it. To get the draw going hold paper or kindleing up close to the damper and light it followed by lighting the rest of the kindling. It is also better to burn a hot fire. Aside from creating a better draw it ensures a clean and more complete burn.
It could be that a tight building envelope is not allowing for enough make up air. You can try cracking a near by window and the radiant heat from the fireplace will still be very cozy. As Angela mentioned the design of the fireplace may need to be improved. Most chimneys are designed at the minimum 2 feet above 10. Building whole chimney higher is costly. A less expensive alternative is to buy a chimney pot which also come in many decorative style.
A chimney brush and extension poles are about the same price as having a chimney sweep out.
