Eeeks! What happened to the jade plant?

Water it every two weeks. So when I went to water it today, this is what I found. This plant has survived four years under my care (neglect?) so I'm not sure what to do now.
It is also cascading because I gave up trying to get the stems up. Broke too many leaves so just left it alone.

It's next to a window. But I move it around sometimes.

Not too much sun, it's a north facing window.

Jade plants need lots of sun or they get leggy. Water it when the soil is dried out at the top, but don't let it go completely dry.

It's hard to tell. Could be a mineral build up. The picture is not clear enough.

Its cascading because it is seeking sunlight. I kind of like it though.
