I love that second photo. I think that the usual way is to have the electrician put the boxes in and then the mirror company measures where they go and delivers the mirror with the proper cutouts.

We just bought a house and there is a large mirror already installed in the master bath. But, the lighting is horrible. Is it possible to cut holes in the existing mirror and then attach the sconces?

No.

No. Glass suppliers do it all the time

Holes aren’t the problem, getting the junction boxes and electric is. Determine the location for sconces, remove the mirror, do the electric, take mirror in or have them come out and cut holes and reinstall, then have the electrician back to add lights.

I did sconces through the mirror when I remodeled the master bath in my previous house. It just took a bit of coordination between GC, electrician, tile guy (because I chose to use a glass tile molding around mirror) and glass/mirror company.

You need to have your sconces picked out so the electrical boxes can be placed at the best height for the fixtures and their function.

It was a newer Spanish style house in a great town, with a great floor plan, but done with "cheap" finishes, so I slowly remodeled it over the years to a more clean/simple style, but still with a Spanish feel to it. Glass tile molding was from Oceanside glass tile. For the vanity it took many days of searching to find a cabinet company that had a Spanish inspired door. Turned out to be 2 towns away, so was a no-brainer to use it. Of course they wouldn't sell the doors to my contractor so I had to pay their price to get the vanity. My GC kept saying he could have built me a vanity at half the price, which was true, but I wanted those doors!

If the sconces stop working is it a nightmare to replace them with the mirror being over the junction box?

NO, not hard to replace old sconces with new sconces. The only issue would be if the problem is the electrical box or wires behind the mirror and not the sconces, which might be harder to fix.

I’m sure yes it would give you a great custom look. Our home was custom built too though. A Texas white and black farmhouse ish look. I miss that property. But not Texas! (Sorry to friends) anyway. Using the same pendant light concept in our new home. An easy way to in the possible future change the entire look with oof the room. And this way was a LOT less expensive. Good luck! Nothing gets chosen in a vacuum. Your other bathroom choices and finishes all play a part if the final decision.
