Installing a flush-mount light fixture

I'm installing a single-bulb flush mount fixture in the bathroom ceiling. Each wire in the junction box has paired leads. So, I have a black wire, a white wire, a black and white wire tied off in one wire nut and two copper ground wires also paired off in a wire nut.

Two questions:

First, since I have two ground wires instead of one, how do I connect the junction box ground to the fixture crossbar?

Second, the new fixture has a thick layer of insulation on top. I see no way to run the mounting screws or center nipple through that insulation. Should I poke holes in the insulation?

The house was rewired three years ago.

Photos show fixture bottom, fixture top and junction box. I'd appreciate any advice. Thanks!
