Take your picture to a local stone tile supplier complete with measurements.

I think it will be very difficult to find a perfect match but good luck. If the home builder supplied these tiles you could check with neighbors to see if they have any extras lying around. If not, see if you can easily remove some tiles from under the fridge without cracking them. If all else fails throw a carpet runner over them.
