Apple scab fungus. Because the tree is infected there is nothing you can do this year, and applying any fungicides will do nothing.

What is the name of the tree? There are apple trees resistant to scab and even a bunch that are immune to the fungus.

Scab is also a disease that does not always require treatment. Many apples can live long and produce well with scab. I had a Gravenstein for many years - very prone to scab - that got it every year (I never sprayed for it) but still remained very productive. Scab rarely affects the fruit other than to make its outward appearance a little funky - perfectly safe and clean to eat or cook with if peeled.

Scab often causes premature leaf drop. If that gets to be serious - consistent seasons can weaken the tree - then that would be cause to spray. Or consider replacement.

Be sure to rake up and destroy all the leaves that may drop now and in fall.

Both those varieties are very resistant - but not immune - to scab (and rust as well). Any infection should be slight.

Yes. That looks like Codlin moth. Cut the fruit in half. Are there tunnels and frass?

No tunnels.

The leaves are already starting to fall off. Is there ANYTHING I can do this year to help? It has been 8 years and every year I have a new issue and never get apples. Birds pecking, drought, this etc.
