Long distance design can be done but you should expect to pay more for the service. With the web it is easy to check out pro designers anywhere in the US. I suggest as a start with NKBA.org ( National Kitchen & Bath Assoc.) for certified kitchen and bath designers. Of course Houzz is a great tool as well for great designers. Find photos of kitchens you love and then seek out those designers to see if they do long distance. Most do not- its much more difficult as it all has to be done by computer and phone. Off the top I do not personally know of any that offer that service, but I am sure there are some out there.
