Judging from the proportions and the fact that there is actual wood burning in there, I'm going to have to say this is an actual masonry wood burning fireplace and not a prefab. It's hard to find a manufactured wood burning fireplace with such nice proportions, but there are several gas models you might consider:

Good point. The 36" size precast masonry fireplaces (FireRock, MasonLite, Isokern) are pretty close to those proportions. But still masonry, not sheetmetal.

It is hard to beat a pre-masonry fireplace kit if you are looking for the masonry look. FireRock has lots of products that when installed look identical to completely custom projects but cost 1/3 the price.
