Basically a recess is cut into the stone (hire stonemason) if it has already been done. It is messy, but not a big deal.

If the purpose is to recess a TV, have the cutout made about 1-2" larger than TV. Have a .5-.75" box built of ply and insert into recess. The TV should be slightly proud of the stone. Finish it off with a picture frame about 3-5" wide to conceal the cutout.
