Vanity Wiring: Sconce and Overhead

Couple of questions...

We are remodeling our bathroom. Including a medicine cabinet that will make an overhead wall mounted light difficult. The cabinet is about 6 inches deep. We need to move the wiring forward to mount the light. Any tips out there for doing that? I have read about it but not in detail.

Also, I would love to also include side sconces but we do not have any wiring to do that and will not be tearing apart the walls to add it.

Can we run wiring down from the light at the top, behind the medicine cabinet and then out to the side from the cabinet and mount the lights? I realize that there will be exposed wire so sort of tacky. But I might find a fixture that could mount flush against the cabinet with an 'arm' off to the side to get the light a bit away from the cabinet. Or just find some way to conceal the wire in a straight tube of some sort.

Or am I just limited pretty much to the one overhead light?
