Does anyone know how I can remove this clean-out cap, or if it's even possible? My plumber says it can't be opened and he may have to just cut the whole pipe and replace it. He didn't try, he was just giving his assessment from looking at it. The cap is 6", the pipe is 4". It looks like there it's been painted over.

Update: We were able to remove it by sawing into the cap and carefully removing it with plyers. Then replacing it with an expansion plug. Thanks for everyone's help.
