I didn't know about a different form... mine is as you describe - small leaves, fairly low growth rate. The tc I tried were weak - I've still got one with two leaves after many years...

Mn Hosta Society must have offered it a few years back, as I saw it in a number of gardens during the 2011 tour. Best one I've seen is below....growing in some light and get horse manure and good watering. Looks close to fit the leaf size you mentioned. No idea on OS vs TC though.

Mine is small. It keeps getting wider each year with new eyes but not any bigger.

I hadn't heard that, it might be possible that a slightly different version went out in TC, but more likely the OS was simply in different conditions so stayed a little smaller. I have a few that stay small but moved one from the same batch out to the wetlands and it is just as large as First Frost now, growing right next to it for the same amount of time.

This is mine... about 12" tall, I would say the leaves are slightly bigger than Golden Tiara, the upper corner is Teaspoon for comparison.
